The film was shot in the USA, Massachusetts. Two islands and a local psychiatric hospital were used for filming. Unfortunately, the lighthouse from the film was not real - it was a 1:12 scale model. The interior shots in the lighthouse were done on a studio set using CGI technology.
Filming Locations:
Island Asylum
In the Movie
Scene where we see the island, the lighthouse, and the shore where the protagonist's ship docks
In Reality
The scene was filmed on Peddocks Island, near Boston. Some forest shots were also filmed here.
Camp "Dachau"
In the Movie
Scene where we see the military camp "Dachau".
In Reality
The scene was filmed at the abandoned Whittenton Mills textile mill complex in Taunton, almost 40 miles south of Boston. A large number of sets, military towers, gates, and barbed wire fences had to be built for filming. There is a moment in the film showing the camp gates: the error is that this is what the gates of the Auschwitz concentration camp looked like, liberated by Soviet troops on January 27, 1945 under the command of Marshal Konev.
Psychiatric Hospital
In the Movie

Scene where Teddy arrives at the hospital on the island
In Reality
The scene was filmed at a real, abandoned hospital in Medfield, Massachusetts. The hospital was repeatedly attempted to be closed, but it was only successfully shut down in 2003. The hospital consisted of 58 buildings built in different styles — they have gradually begun to be demolished, and since 2013 three structures have been lost. Therefore, it is possible that no buildings remain in reality and it will not be possible to visit them.
Teddy's Lake House
In the Movie

Scene where Teddy returns home to his wife and children
In Reality
The scene was filmed at a small lake house in a state park.
